JLMT Application Form - Completion Notes (please read carefully)

1. You must be a current member of the CBSO Youth Orchestra and have completed at least one course (or have left the CBSO Youth Orchestra within the twelve months preceding your application having completed at least one course) to be eligible to apply for a JLMT award.
2. Applications can only be made during the annual application window which is between the CBSO Youth Orchestra's course in October (usually the half term holiday) and 30th November. Applications made outside of this window cannot be accepted.
3. The total value of the JLMT award(s) to be made in 2024/25 will be in the region of £4,500.
4. All information submitted to the JLMT is treated as confidential and will not be disclosed to any person outside of the Board of Trustees.
5. Please ensure that you complete all fields fully prior to submission. Fields labelled with a red asterisk are mandatory.
6. Please do not close this window until your application has been successfully submitted as none of your information will be saved.
7. You may wish to print this application form just prior to submitting it, so you have a copy for your records. You will also receive a copy with your confirmation email after your application has been successfully submitted.
8. Please note that it is your responsibility to provide two references to support your application - see Section 6 below.

The actual details of the applicant’s personal financial situation (or those of their family) are not required, but it would be of great help to the Trustees if the applicant could give a brief outline of the financial circumstances leading to this request below. All information provided will be treated in the strictest confidence and will not be shared with anyone outside of the Board of Trustees.

Two referees will be needed for this application, at least one of whom should be able to write of the applicant’s musical ability and be familiar with the purposes for which the application is made. It will be of great assistance to the Trustees if referees are also able to give an outline of any other circumstances leading to this application.
